Vendor Minimum Security Requirements NYC Health + Hospitals Master Agreement NYC Health + Hospitals BAA NYC Health + Hospitals Data Security Rider NYC Health + Hospitals Cloud Service Security Rider NYC Care RFP 2863 - Budget Proposal Form NYC Care RFP 2863 Invoice Template RFP 2863 - NYC Care Enrollment & Outreach NYC Care (the “Program”) is a health care access service that provides primary, preventive, and specialty care at NYC Health + Hospitals locations, to include NYC Health + Hospitals’ major hospitals and its Gotham Health Centers, as well as other authorized facilities. Eligibility is open to New York City residents, regardless of immigration status, who are ineligible for health insurance or who cannot afford insurance. The healthcare available through NYC Care will be priced on a sliding scale (which may vary across locations), to ensure affordability. NYC Care, through its participating facilities, provides a primary care provider, access to specialty care, behavioral health services, prescription drug benefits, as well as 24/7 customer service to seamlessly connect patients to healthcare. After launching in August 2019, NYC Care currently has an enrollment of approximately 125,000 patients, and the Program reflects an investment by the City of at least $100 million annually in care provided to New Yorkers.
